National referral centre for familial young onset and unusual dementias

Multidisciplinary clinic with specialist nurse, psychology, neurology and psychiatry input plus Alzheimer’s Society representative

Integrated rapid access slots in both Monday and Tuesday clinics provide a one-stop assessment with medical assessment , investigations including MRI, EEG, psychometry, bloods, CXR and ECG. Nurse assessment is also provided.  A specialised lumbar puncture clinic operates in parallel and accelerates speed of investigation and diagnosis. Patients will return approximately four weeks later for results and discussion of diagnosis.

We liaise directly with the GP and with the patient. Results are communicated in person in clinic, by letter to GP or by telephone if urgent.

Referrals are accepted from primary, secondary and tertiary care.

Requirement – please complete and attach score-sheet-gad-7 and GPCOG forms if there is any history of anxiety/depression.
