A Treatment Summary is a document completed by a member of the team after the end of a phase of your treatment (not necessarily at the end of all your treatments).

Your Treatment Summary will include details of the treatments/surgery you have had, potential outcomes/consequences of the treatments/surgery, and signs and symptoms that may indicate the possibility of recurrence.

The Treatment Summary will be shared with you and your GP. The Treatment Summary provides your GP with up to date information and will inform them of any actions that need to be taken by them. It also helps them to better understand the treatments/surgery you have had to help them support you.

If you haven’t been offered a Treatment Summary, please ask your Clinical Nurse Specialist (CNS) or one of the health care professionals involved in your treatment.
