Coming to the clinic

When you come for your appointment, please bring

  • a list of all your medicines, including any you buy from the pharmacy

  • your mobility aids, splints and wear loose clothing so we can look at your arms and legs easily

  • any communication aids you may use and your hospital passport

  • any letters from other people who have given you medical care

  • names and contact details for anyone involved in your care, such as your GP, OT, Physio or Wheelchair Services

  • you are welcome to bring someone with you if you wish, such as a family member, carer or friend

During your appointment
