Our experienced multidisciplinary team works closely with your local hospital to make sure your care is well-coordinated. This helps make sure you get the right support throughout your treatment, including any follow-up care closer to home.

2-Week Wait Referral

All 2-Week Wait prostate cancer referrals must be sent via e-referrals to:‘2WW - Suspected Prostate Cancer - Triage Service - UCLH – RRV’

We cannot accept 2-Week Wait referrals via email or post.

Tertiary Referrals (Second Opinion / Specialist Treatment)

For tertiary referrals, please ensure you submit all the required information only to:ucl-tr.uro-ca-ctr@nhs.net

Minimum Data Set (MDS) Requirements:

  • Inter-Trust Referral Form (ITR) (also known as MDS or Tertiary Referral Form) – if unfamiliar, please ask your MDT team to arrange.
  • Referral Letter
  • Imaging Reports (i.e., MRI, Bone Scan, PSMA)
  • Pathology Report
  • MDT Outcome

Incomplete referrals may result in delays.
