All 2-Week Wait prostate cancer referrals must be sent via e-referrals to:‘2WW - Suspected Prostate Cancer - Triage Service - UCLH – RRV’
We cannot accept 2-Week Wait referrals via email or post.
Tertiary Referrals (Second Opinion / Specialist Treatment)
For tertiary referrals, please ensure you submit all the required information only to:ucl-tr.uro-ca-ctr@nhs.net
Minimum Data Set (MDS) Requirements:
- Inter-Trust Referral Form (ITR) (also known as MDS or Tertiary Referral Form) – if unfamiliar, please ask your MDT team to arrange.
- Referral Letter
- Imaging Reports (i.e., MRI, Bone Scan, PSMA)
- Pathology Report
- MDT Outcome
