MyCare UCLH is our patient app and online service. Use MyCare UCLH on your mobile (through the MyChart app) or computer (in your browser) to securely view and manage information about your health and care at UCLH. With MyCare UCLH, you can access details about your past and upcoming care at UCLH. You can take a more active role in managing your health by viewing clinic letters and test results, communicating with clinicians, completing pre-visit tasks or attending video appointments.
Scope of Guidance
The MyCare Quick Start Guide gives a patient-friendly overview of how MyCare UCLH, our patient portal and app, works and includes guidance on the most used functions. MyChart app has a similar layout to our website, so the instructions in this guide will apply to both web and mobile versions.

Common Terminology
MyCare - joint name for UCLH patient web services and app.
MyChart - the name of the app on Google Play store and Apple Store. MyChart and MyCare app are used interchangeably.
After Visit Summary (AVS) - a summary of the care you received during your visit that may include a list of medications, care instructions, allergies and more.
Appointment Letters - inform you of the date and time of upcoming outpatient clinic appointments as well as contain any special instructions that you might need to follow before that appointment. You can find appointment letters in the Appointment Letters menu in MyCare.
Clinic Letters - summaries of outpatient appointments; hospital stays or explanations of test results that are written by healthcare professionals at UCLH. Usually both your GP and you will both receive a copy of your clinic letter.
Proxy - a proxy refers to a person who is granted access to another patient’s MyCare account. This allows the proxy to view and manage parts of the patient’s medical information and care on their behalf.
Share Everywhere - is a feature in MyCare that allows you to securely share your health information with any healthcare provider, even if they don’t use the Epic system.
Happy Together - is a feature in MyCare that allows you to view information of your medical record from another organisation that uses Epic.

You can access MyCare by using the internet browser on your computer at https://
You can also access it through an app on your mobile. The mobile version of MyCare UCLH runs on the MyChart app provided by Epic. You can download the MyChart app free of charge from the app store at the links below.
If we have sent you a personal activation code on your After Visit Summary, by letter, email, or text, you can quickly activate your MyCare account on the MyCare UCLH - signup page.
If you do not have a personal activation code, or if you want to request a MyCare UCLH account before your visit, you can request this online with your NHS number using the link here.

When you log into MyCare, you are welcomed by our homepage which allows you to conveniently navigate between different activities, as well as personalise your page view. The homepage contains the main MyCare Menu (A), Shortcuts (B), Health Feed (C), and Explore More field (D). Below, are described the main ways to navigate MyCare.

A) The MyCare Menu is the main way to navigate between different activities on the web version and the app. Each individual option in the menu is explained in this guide.

B) Shortcuts toolbar provides patients with quick and easy access to the activities they use most frequently. They are mostly based on the patient usage data.

By clicking on the Cog icon, patients can personalise what default shortcuts are shown in their homepage.

C) Health Feed is a unified view of actionable health updates for the patient. The cards in the Health Feed are generated automatically and sorted by priority, with the most important items appearing at the top. These cards may include important announcements, upcoming appointment preparations, after-visit summaries, test results, and more.

D) Explore More field, which appears at the bottom of the health feed, may contain information about new MyCare functionalities and links to other useful tools.

MyChart app has a similar layout to our website, so the instructions in this guide will apply to both web and mobile version.

View Care Team
View Care Team feature provides details of certain clinicians assigned to your care.

Please note, not all clinicians that the patient may have seen will be listed in this feature. Currently most clinicians providing care in outpatient encounters will not be viewable in this feature, therefore it is not unusual to see no clinicians listed here.

One of the most important features of MyCare is the ability to actively engage with your care team. The Communication topic is what allows patients to read messages sent by their care team, view letters, and ask questions.

Messages
You can read messages sent by your clinician or other hospital staff by going to the Message Centre (Communication > Messages) in the Menu or by clicking on the Messages topic in your homepage shortcuts.

The Message Centre contains five subfolders:
- Conversations - This subfolder contains messages related to conversations with clinicians, questionnaires you have been asked to complete, messages about test results, and more.
- Bookmarked - To flag a conversation for easy access in the future, open the message and use bookmark button. This message will then be moved to the Bookmarked subfolder.
- Trash - alternatively, you can click on the Move to Trash button to move the conversation to the Trash subfolder. If needed this can be undone by clicking on the Restore Conversation button.

Appointment Letters - this subfolder conveniently stores all your clinic appointment letters in one place. Appointment Letters inform you of the date and time of upcoming outpatient clinic appointments as well as contain any special instructions that you might need to follow before that appointment. Below is an excerpt from an example clinic letter.

- Automated Messages - this subfolder stores all the system messages sent to you automatically, such as appointment reminders, questionnaire submissions, health tracker reminders and more.

If you have a non-urgent medical question, you can send a message one of the services currently managing your care at UCLH. You can access this feature in the Menu (Communication > Ask a Question) or from the Message Centre (Communication > Messages) by clicking on Send Message button.

Messaging functionality is currently limited at UCLH. As a result, you may see a disclaimer indicating that no recipients are available. For more information on how to send messages, please see our page View and send messages with MyCare.
Clinic Letters
Clinic Letters are summaries of outpatient appointments; hospital stays or explanations of test results that are written by healthcare professionals at UCLH. Usually both your GP and you will receive a copy of your clinic letter.
You can access all your clinic letters sent by your team in the Menu by following Communication > Clinic Letters.

The My Record submenu stores key features related to your ongoing care. It provides access to important health information such as your appointments, test results, medications, allergies, immunisations, health conditions, family history, and more.

Appointments (Upcoming Visits)
You can view your past and future appointments by navigating to My Record > Appointments or by clicking on Appointments shortcut on the homepage.

A) Your upcoming appointments will appear on the top of the page:
- Seven days before an appointment, Appointment Preparation opens, allowing you to complete medication, allergy, and health issue reviews in advance.
- Appointment Preparation also lets you complete any applicable questionnaires.
- Select the Details button to view the date, time, location, and the type of your appointment.
If you have any existing medications, allergies, or health issues, they will appear in the Appointment Preparation for review. You can review each item, click the plus button to add new entries, or request to have them removed. Use the Next button to navigate through each section.
Completing Appointment Preparation in advance allows your clinical team to review the information beforehand and better prepare to provide you with the best care.
Please note: any information entered here will have to be manually reconciled by your clinical team during your appointment before it becomes part of your medical record.

Some appointments will require you to fill out additional questionnaire(s) related to your condition. If that is the case, the questionnaire will be available in Appointment Preparation, or in their separate Menu feature My Records > Questionnaires (see "Questionnaires" below).

Video Appointments
Your upcoming appointment may be in person or a video visit, which means you can join using the MyCare web version or app. It will be clearly marked as a video visit under the appointment type in the Upcoming Appointments section (My Record > Appointment).

You can complete Appointment Preparation for these visits as described above. You can also click the Details button to see more information and instructions for your appointment.
The Join Video Visit button will become available 15 minutes before the scheduled appointment start time. Click this button to join the call.

Depending on your communication preferences, you may receive a reminder via the MyCare app and/or email, which will also include a button to join the call.

Appointments (Past Visits)
Your past signed appointments will appear at the bottom of the page. A signed visit is one where your clinicians have completed all necessary documentation for that visit and have indicated in our system that it is finalised.

Most of the past visits will have an After Visit Summary (AVS) available. Your AVS may include your upcoming booked appointments, medication list, after care instructions, and any topics discussed during your appointment.
AVS documents are generated dynamically, so the amount of information included depends on what was documented during the appointment.
You can view and/or download them by clicking on View After Visit Summary button.

Some past visits may also include separate clinical notes shared with you by your clinical team. You can access these by clicking the View Clinical Notes button.
Please note: In most cases, your past visit details and notes are in a form of a clinic letter, which you can access in Communications Menu, rather than in the notes found in this section.

Test Results
With MyCare, you can view certain test results online. The timeframe for when results become available depends on the type of test. After your clinical team receives the results, they may need time to interpret them. Occasionally, results are reviewed by a larger clinical team to help design the best treatment plan for you. This process can extend the time between your test and when you receive the results. We expect our clinicians to provide explanations alongside the results to help you understand what they mean for your care.
- Basic outpatient blood test results become available 24 hours after being finalised.
- Inpatient test results usually become available three weeks after discharge.
- Certain outpatient imaging results and some more complex laboratory tests become available three weeks after being finalised.
Please note that not all types of results are released automatically. Some results must be manually released by a clinician.
You can access your test results either by clicking the Test Results shortcut on the homepage or through the menu (My Records > Test Results).
The results are listed in chronological order and can be searched by using the bar at the top. Simply click on the result you’re interested in to see more details.

Once you click into a result, you can:
A) See the normal range for that particular result.
B) View the interpretation and any comments.
C) Learn more about the test- this will direct you to an external page where you can read more about the test.
D) Compare overall result trends for that result entry (Compare Result Trends button) and view trends for a specific result (View Trends button).

You can use the timeframe buttons above the graph to adjust the view, and the filters on the right to focus on a specific lab result. Please note that the result trends feature may not be available for all types of results.

Medication
You can view all your current medications in one place by navigating to My Record > Medication in the menu. For each medication, you can see details including the prescribed dosage, instructions, and the prescribing clinician. Please note that any medications administered during your inpatient hospital stay will not appear here.
If you are no longer taking a prescribed medication, you can click the Remove button and add a comment explaining the reason. Your clinician will review this change at your next appointment.
You can also use the Related Links on the right to jump to other activities and review records such as allergies, immunisations, and health issues.

Health Summary
To get a summary of many aspects of your UCLH medical record, go to My Record > Health Summary. This central hub provides an overview of your care and includes useful links to related activities. It contains sections such as health goals, medications, test results, current health issues, allergies, and quick links.

This feature may contain preventive care reminders relevant to your care.
Please note that this feature is not yet widely deployed at UCLH, so if it appears blank, this is expected.

Questionnaire
Your clinical team may make questionnaires available to you via MyCare so you can complete them online before your appointment. The questionnaire answers will be discussed with you during your visit.
- If the questionnaire is associated with an upcoming visit, it will usually become available about 7 days before the appointment and can be accessed via Appointment Preparation (discussed on page 25) or the My Record > Questionnaire.
- Sometimes clinicians may send you a questionnaire even if you do not have an upcoming visit. If the questionnaire is not associated with a specific visit, you can access it through the My Record > Questionnaire or via Messages (Communication > Messages).

You can view any upcoming outstanding procedures, tests and patient initiated follow up (PIFU) requests by following My Record > Upcoming Tests and Procedures.

Medical and Family History
Your medical and family history recorded at UCLH can be viewed by navigating to My Record > Medical and Family History.
Here, you will find sections for surgical, family, and social history. Please note that not all your medical history may appear in MyCare. If information has not been recorded at UCLH, it will not be available here.

Track Your Health
Your clinical team may ask you to record information such as daily steps or blood pressure readings in MyCare. If so, you will find a Track My Health feature available under the My Record submenu. When you enter this data using the Track My Health feature, your clinician automatically receives the readings and can monitor your progress between visits.
When you get a new health tracker assigned, you will receive an automated message.

The message will contain more information about the specific tracker assigned to you. You can access the tracker by clicking on the hyperlink, or you can access it directly via My Record > Track My Health.

The Link Account button takes you to a detailed view of options for automatic tracking using third-party apps. Linking your accounts is optional but can be more convenient, as it eliminates the need to manually enter data into the tracker each time.

If you wish to enter tracking data manually, you can click on each individual tracker in the Track My Health option and select Add Readings. This view also allows you to see trending data over different time periods. You can do this both on the web version and the app.

Document Centre
If you would like to download parts of your medical record at UCLH or request certain legal records such as coordination of care or government reporting, you can do this in the Document Centre (My Record > Document Centre). Please note, you cannot request access to your full medical record via MyCare. To do so, please contact uclh.

A) View or Download Visit Records
This option allows you to download your healthcare information related to your visits. If you choose this option, you can filter the information by date or specific visit and download it to your computer or phone (if using mobile version).

B) Requested Records
This section contains all the records you have previously requested, either through the View and Download Visit Records option or via the UCLH Release of Information Department.
Please note that certain types of document releases have an expiry date. These will appear separately, and once expired, they will no longer be available to download.

C) My Documents
Certain types of documents uploaded by your care team, such as GP letters, will appear in the My Documents section of the Document Centre. Consent forms requiring your signature will also be available in this section. If this applies to you, you will be notified according to your communication preferences.

D) Who’s Accessed My Records?
Here, you can see a list of MyCare users or third-party apps that have accessed your records. This is mostly relevant to patients who have granted proxy access to a family member or carer, or who have connected a third-party app, such as Google Fit, to their MyCare account.

The Sharing submenu primarily focuses on the various ways you can use MyCare to share your medical information with other healthcare organisations, friends, or family. Some options within this section will direct you back to the Document Centre, where you can download specific information and share it as you see appropriate.

If you need to provide your healthcare information to a non-Epic organisation, GP, your workplace, or your insurance provider, you can download a copy of your health record from Sharing > Sharing Hub.
If you are unsure which sharing method to use, you can choose from four options: Yourself, Family, Healthcare Provider, or Anyone Else. These options are designed to guide you toward the most appropriate sharing methods for each category.
However, if you are already familiar with the available options, you can skip ahead and select the most appropriate choice from the bottom of the page.
A) Manage Friends and Family Access
If you have granted someone proxy access to your MyCare account, this option lets you view and manage who can access your records. You can remove access for a particular proxy by clicking the Remove button.

B) Grant One Time Access with Share Everywhere
Share Everywhere is a way for you to share your medical information with your care team outside of UCLH. Using your MyCare or MyChart mobile app, you can generate a share code and provide it to the person you want to share your health data with.
The recipient enters the share code along with your date of birth on the Share Everywhere website to gain one-time, temporary access to your health information. They can also write a note back to your chart to help keep our UCLH team informed about the care they provided.
You can access this feature by clicking Grant One-Time Access with Share Everywhere in the Sharing Hub or separately via the menu Sharing > Share Everywhere.

C) Download Health and Visit Summary
Clicking this button will take you to the Document Centre (discussed on page 31), where you can filter your health information by dates, appointments, or select all records. You can then download the information and share it as you wish.
D) Request a Formal Copy
This option is not available as patients should follow a formal Release of Information procedure.

E) Give Permission to Share Your Health Record
Currently UCLH shares certain medical information with other Epic-affiliated healthcare organisations in the UK. To simplify access to your health records, you can share your Care Everywhere ID if / when visiting these organisations.

F) Download Brief Health Summary
This feature allows you to request a basic brief health summary that may contain your demographic information, vitals, medications, immunisations and more.
Simply tick the acknowledgement box and select Request download.

Once the request is complete, click Go to Requested Record to download the document to your computer or mobile device.
Link My Accounts
If you have received care at another healthcare organisation, you may be able to view information from that medical record directly in MyChart. This feature is referred to as Happy Together. The information you might see from other organisations includes:
- Allergies
- Care team
- COVID-19 vaccines
- Health issues
- Medications
- Messages
- Test results
- Visits
To view this information, you must link your account. Go to Sharing > Link My Accounts to get started. Simply search for the organisation, select it and click Link Account button.

The Resources submenu provides access to a range of helpful tools and information from both UCLH and the NHS. These may include links to trusted health information, hospital services, patient education materials, support resources, and guidance on managing your care.

Search Medical Library Feature can be used to search for medications, procedures, or medical conditions. The search will take you directly to the NHS website.

If you are enrolled into a research study at UCLH, the relevant resources can be found in the Research Studies feature (Resources > Research Studies).

UCLH Patient Information
The Resources > UCLH Patient Information option will take you directly to the UCLH website, where you can access a wide range of patient information leaflets. These leaflets have been developed by various specialist teams across UCLH and cover a broad range of health conditions, treatments, procedures, and services. You can search the resources by keywords and/or specialties.

The Settings submenu contains a range of options that allow you to manage and personalise your MyCare experience. Here, you can update your personal details, adjust your communication preferences, and customise how the app or website appears and functions for you.
These settings help ensure your information is up to date and your experience is tailored to your preferences.

Personal Information
Here, you can update your basic contact details, demographic information, and emergency contact details.
- To update your demographic details, click the Edit button. This information can also be updated via Settings > Other Preferences.
- To add an emergency contact, click Add Contact.
Please note: Any changes to your demographic information must be manually reconciled by the UCLH team, so updates may not take effect immediately.

The Account Settings feature allows you to manage several important aspects of your MyCare account to ensure your experience is secure, personalised, and convenient.

A) Update your login credentials - Change your password and manage passkeys to help keep your account secure.

B) Manage your two-step verification settings - Two-step verification is a requirement for our digital systems and increases your MyCare account security. To make sure the person logging in is you, we will email you a unique passcode when you login.

C) Manage communication preferences - Choose how you would like to receive alerts and updates (such as via email, post, SMS, or in-app notifications) about appointments, test results, or messages. You can access Communication Preferences via Settings > Account Settings or separately via Settings > Communication Preferences.
Simply select the type of communication on the right, then toggle the alerts and notifications for each option according to your preference.

D) View and manage connected devices - remember browsers you have used to access MyCare and remove any you no longer use or recognise.
You can choose whether MyCare should remember the devices you use to log in by toggling the option below. This feature adds convenience while helping you maintain control over your account’s security.

If you wish to see a list of remembered devices click on Remembered Devices option. To remove a device from the list, you can click on Forget Device option.

You can alternatively access this setting separately through Settings > Linked Apps and Devices.
E) Manage your account - Deactivate the app if ever necessary.
If you choose to deactivate your MyCare account, you will be logged out and lose access to all MyCare features. You will no longer receive messages directing you to log in, and any linked accounts at other organisations will no longer display your data from UCLH. Deactivating your account does not delete your medical records, which are retained for legal and clinical reasons. If others have proxy access to your records, they will continue to see your information. You can rejoin MyCare at any time. To reactivate your account, contact your healthcare provider.

Personalise
The Personalise feature allows you to customise your MyCare experience by choosing a colour scheme and adding a nickname for yourself as well as for any accounts you have proxy access to. You can also use this section to revoke proxy access if needed.
